Living Solutions for Seniors will guide you through the maze of senior service
resources available in the greater Austin Area. Helping your elders maintain the
lifestyle they are accustomed to is the goal of Suzanne Sherman RN. This is done
by this utilizing Geriatric Case Management.
Case management is an assessment of the physical, social, medical, spiritual,
financial, and legal status of the senior. Their caregivers' needs are also assessed.
Living Solutions for Seniors then provides you with a personalized list of options
to choose from.
You will also be assisted in putting your choices into action. I will provide
needed services or arrange intake by chosen providers. An evaluation of the
services id done to insure your goals are being met. Any additional
recommendations are then discussed.
The goal of Living Solutions for Seniors is to have these needs fulfilled in the
most cost effective manner yet maintaining a desired style of living for your
senior family member; safety being the priority of any choices. By adopting a
holistic philosophy of care management I remain focused on the client as an
individual.
